Flu Shot Clinic at LMHS Friday Oct 23rd

The Lincoln County Public Health Department will be holding a flu shot clinic at LMHS on Friday, Oct 23rd. If you would like your child to receive a flu shot, please complete the attached form and have your child turn it into the office before the clinic. Forms are also available from the office. Your child will not be given a flu shot without the completed form.
