10-12th Graders - Last Year's Sports Physicals Good for This Year, Only Interim Form is Needed
Very Important Update About Sports Physicals--Please Read--
Athletes who are going into 10th -12th grade this year who HAD a physical completed LAST year and uploaded it to Arbiter DO NOT need to get a new sports physical this year. Instead, they and their parent/guardian just need to complete the attached, one page Interim Medical Form from MHSA, and upload that to their Arbiter registration where the sports physical usually goes.
Athletes who are going into Grade 9 (or lower) MUST get a new sports physical this school year and upload it into their Arbiter registration where indicated, whether or not they had a physical last year.
NOTE ABOUT AUG 6th SPORTS PHYSICALS DAY @ LMHS: The times posted yesterday in the last Parent Square post were the correct times CHC will have physicals here tomorrow. There was an incorrect flyer in the Back-to-School folder you may have seen, and we're working on replacing it with the correct flyer. CHC will do sports physicals tomorrow in the following time slots by last name:
- A–F: 9:00–10:30 AM
- G–L: 10:30 AM–12:30 PM
- M–R: 1:00–2:30 PM
- S–Z: 2:30–4:00 PM
