Sports Physicals Day - August 6th - LMHS Gym (Corrected Info)

August 6th is CHC Sports Physicals Day at LMHS in the Gym (Corrected Info)

REQUIREMENTS FOR PHYSICAL:

• Either the parent/guardian must come with their child to the LMHS Gym for the physical,
• OR the parent/guardian must fill out and sign the permission slip AND fill out the questionnaire part of the physical itself with their child, and both parent and child must sign the physical form. The child must then bring both forms when they arrive to get their sports physical completed by CHC on August 6th. Both forms are attached to this post. 

Sports Physical Time Slots by Last Name: 

• A–F: 9:00–10:30 AM
• G–L: 10:30 AM–12:30 PM
• M–R: 1:00–2:30 PM
• S–Z: 2:30–4:00 PM

All athletes must also be registered on Arbiter Sports with physicals uploaded (arbiter.io). Must use a personal email address, not a school email to create accounts. A complete registration form must be filled out and submitted. There will be space to upload your physical during the registration on Arbiter. The school does not need a copy of your sports physical, but we can help walk you through the registration process if you need assistance or WiFi access.
